Monthly Summary Report

Monthly Summary Report

Begin by toggling the button to indicate that the PHC’s monthly summary report for the month you’re recording data has been completed and is available.
Record patient attendance data for the month by selecting the appropriate clinic and gender, then input the number of patients attended. You can add additional attendance records by clicking the “+” button.
Proceed to enter clinical metrics by selecting the relevant metric and entering the data.
To record the observed attendance, count the number of patients you see physically at the time of your visit. Choose the particular clinic and enter the corresponding number of patients observed, grouped by gender.
Toggle the button to initiate the patient feedback survey. Ensure that you ask the requisite number of patients the question: “How satisfied are you with the service being offered?”
