This section verifies whether you are physically present at the assigned PHC and collects a visual record.

Toggle the “I am at the specified facility” button above the map interface.

This uses your GPS to confirm physical location.

Scroll down and take a live photo of the PHC using the device’s camera.

Retake if unclear.

Uploads of pre-saved photos are not allowed to ensure real-time evidence.

Click Continue to proceed.
